Published in the Lancet Psychiatry, this study marks the first major investigation into what some term "AI psychosis," suggesting that chatbots can validate or amplify delusions, though likely only in those with pre-existing susceptibility.

Dr. Hamilton Morrin, a psychiatrist and researcher at King's College London, led the analysis by examining 20 media reports on AI-induced psychosis. He identified three primary categories of psychotic delusions: grandiose, romantic, and paranoid. Chatbots, with their sycophantic responses, tend to latch onto grandiose delusions, often using mystical language to imply users have heightened spiritual importance or are communicating with cosmic beings.

Morrin noted that media reports became crucial in his work, as he and colleagues observed patients using large language model AI chatbots that validated their delusional beliefs. "Initially, we weren't sure if this was a widespread issue," he said, adding that by April last year, reports emerged of individuals having delusions affirmed and even amplified through AI interactions.

While terms like "AI psychosis" are gaining traction in outlets such as NPR and the New York Times, Morrin advocates for more cautious phrasing, such as "AI-associated delusions." This reflects the lack of evidence linking chatbots to other psychotic symptoms like hallucinations or thought disorders. Many researchers, including Dr. Kwame McKenzie of the Center for Addiction and Mental Health, believe AI is unlikely to induce delusions in people without pre-existing vulnerability.

Dr. Ragy Girgis, a professor at Columbia University, warned that chatbots could worsen "attenuated delusional beliefs," potentially leading to irreversible psychotic disorders. He emphasized that the interactive nature of chatbots might speed up this process, as noted by Dr. Dominic Oliver of the University of Oxford.

Industry Responses and Safeguards

OpenAI stated that ChatGPT should not replace professional mental healthcare and highlighted collaborations with 170 mental health experts to enhance safety in newer models like GPT 5. However, even these advanced versions have given problematic responses to mental health crisis prompts. Anthropic did not respond to requests for comment.

Creating effective safeguards is challenging, Morrin explained, as directly challenging delusional beliefs can lead to social isolation. Instead, a balanced approach that understands the source of delusions without encouraging them is needed—a task that may be beyond current chatbot capabilities.

Morrin pointed out that people have used media to reinforce delusions long before AI, but chatbots offer a faster, more concentrated dose of validation.
